SEREN Connect

SEREN Connect is a structured education programme delivered by healthcare professionals to support living with diabetes.

Resources are used as part of 1:1 discussion and group sessions when preparing to move to adult services. The sessions move through topics supported by a series of booklets and e-booklets, all of which are available in English and Welsh.

Additionally, in response to COVID-19, SEREN Connect has been partly adapted for virtual delivery; virtual sessions are likely to be an ongoing delivery option.

The group education element of the SEREN Connect programme is split into four education sessions and covers the key themes as outlined below. 

  • Normalising living with Type 1 diabetes
  • Getting out and about
  • Learning to drive
  • Telling new people.
     
  • Alcohol
  • Sex and relationships
  • Moving to adult services
  • Understanding diabetes care.
     
  • Managing stress
  • Accessing support
  • Strengths and skills
  • Work and employment.
     
  • Holidays
  • Travelling, events and festivals
  • Becoming independent
  • Any additional/optional topics.
